Put the file in your server's main folder (next to server.properties) and restart. The name must be exactly server-icon.png.

Server icon requirements

Exactly 64 × 64 pixels, PNG format.
File name server-icon.png, all lowercase.
Placed in the server's root folder, then restart the server.

FAQ

My icon doesn't show up. Why?

Check the size is exactly 64×64, the file is a real PNG (not a renamed JPG), the name is server-icon.png, and that you restarted the server. Some clients cache the old icon; remove and re-add the server in your list.

Does Bedrock Edition use a server icon?

No. Bedrock Dedicated Server doesn't show a custom icon in the server list. This file is for Java Edition.
